The DNS stated she was unaware of the concerns until surveyor inquiry and said the complaint should have been completed when first voiced. Review of grievance forms found no grievance for Resident #96 regarding inappropriate staff behavior, despite the facility policy requiring staff to record verbal grievances and forward them to the Grievance Official as soon as practicable.
