Infection Control Lapses in Linen Storage, Specimen Handling, and Contact Precautions
Summary
The facility failed to provide and implement an infection prevention and control program when clean linen, utility rooms, specimen storage, and contact precaution practices were observed to be out of compliance with facility policy. On the south wing, a linen cart was observed with its front flap open and clean linen exposed, while a Monster beverage and a personal water bottle were inside the cart. The charge nurse stated the beverages should not have been inside the clean linen cart and removed them. Facility policy stated clean linen is to be handled to prevent re-contamination and stored in a separate designated room with a closed door, and that clean linens are not to be placed on the floor or other contaminated surfaces. In the north wing soiled utility room, two trash bins without covers contained closed bags of trash, and a specimen refrigerator on a high shelf had no lock, a thermometer was present, but there was no temperature log; two specimens were awaiting pickup. Numerous pillows were also kept on a shelf within the dirty utility room. In the clean utility room, mechanical lift pads were hanging vertically with the lower portions touching the floor, two closed bags of pillows were on the floor, a small linen cart was uncovered, and an open brief bag was in the sink. Additional observation showed the south dirty utility room also lacked covered garbage containers and had a specimen refrigerator with a thermometer but no logbook. The nursing supervisor was unsure whether specimen refrigerators needed to be locked when specimens were inside, and the DNS observed the conditions and stated nothing should be touching the floors. Resident #25 had diagnoses including sepsis and C. diff, was cognitively intact, and had a positive C. diff specimen result. The resident’s care plan included Contact Precautions. During a blood sugar check in the resident’s room, the RN doffed PPE after hand hygiene, but no trash receptacle was available for disposal of used PPE at the doorway; only a small individual trash container was noted at the bedside. The DNS stated a trash receptacle should have been located just inside the room’s doorway and indicated staff were being in-serviced about the need for a receptacle in rooms of residents on contact precautions. The facility policy on Transmission Based Precautions addressed donning PPE before entering the room but did not describe how used PPE was to be disposed of after doffing.
