Menu substitutions were not posted or communicated to residents

Complete Care At Fox HillVernon, Connecticut Survey Completed on 03-03-2026

Summary

The facility failed to ensure that menu substitutions were posted on the daily menu before mealtime and that residents were informed of changes to the noon meal. During an observation and interview on 2/24/2025 at 12:10 PM, the Regional Dietary Manager stated the kitchen was out of peas and was substituting cabbage and spinach because a delivery had not been received the prior day due to a snowstorm. The daily meal list was posted outside the main dining room, but the substitutions had not been added to the menu, and no staff were observed communicating the change to residents in the dining room. During breakfast steam table service on 2/25/2026 at 8:25 AM, the menu listed biscuits and gravy, but no gravy was served on the steam table and corn bread was being served instead. The Dietary Manager stated there were not enough biscuits, so corn bread was made, and the cook stated gravy had been made but was not visible on the steam table and residents would have to ask for it if they wanted it. A service line checklist completed that morning did not include gravy. Later that morning, residents stated they were not being notified of menu changes and were not being served what was on the menu.
