Failure to Protect Resident During Bed Repositioning

Summary

The facility failed to ensure adequate supervision and protection from accident hazards during in-bed repositioning for one resident who required substantial assistance to roll in bed. The resident was admitted with multiple diagnoses including COPD, respiratory failure, cellulitis of the lower extremities, diabetes, cirrhosis of the liver, and heart failure. The MDS showed moderate cognitive impairment, dependence on staff for hygiene, bathing, dressing, and transferring, and substantial assistance needed for rolling left to right in bed. The resident’s care plan identified the need for assistance with ADLs because of impaired balance, dizziness, recent hospitalization, COPD, diabetes, morbid obesity, rheumatoid arthritis, glaucoma, and pulmonary hypertension. The care plan also stated that two staff members were to be present at all times when entering the resident’s room. The resident also had physician orders for oxygen at 2 LPM continuously, hospice evaluation and treatment, and hospice admission for COPD. A hospice note documented the resident was awake, oriented, and doing well shortly before the incident. During incontinence care, a CNA elevated the resident’s bed and repositioned her onto her side. The resident rolled off the elevated bed onto the floor. The incident report and nursing notes documented that the resident had a change in level of consciousness after the fall, with decreased oxygen saturation and increased blood pressure. Staff interviews confirmed the resident was on the floor after rolling out of bed during care, that the bed had been elevated, and that the bed had been moved away from the wall. The resident reported head pain after the fall, was transferred to the hospital within about an hour, experienced cardiac arrest en route, and died shortly after arrival at the hospital.
