Unlabeled, Undated, and Expired Food in Nourishment Refrigerators

Summary

The facility failed to ensure food was prepared, distributed, and served under sanitary conditions in the two nourishment refrigerators. Surveyors found multiple food items that were unlabeled, undated, or expired in the 300 hall and 600 hall nourishment refrigerators and freezer, including pitchers of red, yellow, and white liquids, thawed Uncrustable sandwiches, juice bottles past expiration, grapes with wrinkled skin and brown stems in brown fluid, chocolate milk with brown chunks, resident pizza boxes, and health shakes without pull dates. The observations showed that food items were being stored in the nourishment refrigerators without the required labeling and dating, and several items appeared spoiled or expired. On one unit, a resident's personal food containing sliced meat had a film covering the meat and dripping fluid from the container, and multiple thawed Uncrustable sandwiches were present without dates. On the other unit, surveyors observed expired juice bottles, undated grapes, expired chocolate milk, undated Uncrustable sandwiches, and several resident pizza boxes with dates ranging from 3/29/26 to 4/21/26. During interviews, an LPN stated the night shift was supposed to clean out the nourishment refrigerator/freezer. The NHA stated CNAs should be cleaning out the nutrition refrigerators as needed and that all items should be labeled and dated, with expired food cleaned out. The DM said he was not sure who should be cleaning out the nourishment refrigerators, dietary staff were responsible for taking drinks and snacks to the units, and it was important to clean out the refrigerators so residents were not served expired foods or drinks. The DM also stated all items needed to be labeled and dated and that Uncrustable sandwiches should not be in the refrigerators longer than three days.
