Delayed Resolution of Grievance for Missing Eyeglasses
Summary
The facility failed to ensure a prompt resolution for a grievance involving one resident’s lost prescription eyeglasses and eyeglass case. The resident was admitted with vascular dementia and combined forms of age-related cataracts, and the admission inventory documented that she had a pair of glasses. The theft and loss log and theft and loss record showed the eyeglasses were reported missing, and a social services note documented that staff were notified the glasses could not be found. The resident’s family member later provided a copy of the eyeglass prescription and stated the resident needed replacement glasses because the current pair had been lost for months. The family member reported that the facility had not resolved the grievance for over five months and said the facility kept trying to obtain insurance authorization instead of replacing the glasses. The family member also stated the receipt for the glasses had been provided to the facility. The LTCO stated the length of time taken to replace or reimburse the glasses was inappropriate, especially since a current prescription had been provided months earlier, and indicated an expected resolution time was approximately one month. The LTCO further stated the facility should either replace or reimburse the item with something comparable, only after approval from the resident or family member. Facility staff confirmed the grievance remained unresolved. CNA staff stated lost items were to be searched for and then reported, and LN staff stated missing items were reported to social services and that grievances could be filed on a form at the nursing station. The SSD stated social services handled the grievance process, but final approval for replacement or reimbursement came from the Administrator, and confirmed the resident’s glasses had still not been replaced or reimbursed after several months. During review of the grievance forms, the SSA and SSD confirmed the grievance for the missing eyeglasses and case had not been resolved and stated they were unaware that a written summary of the investigation needed to be provided to the person who filed the grievance. The facility policy required investigation, administrator review, informing the complainant of findings and actions, and providing a written summary of the investigation.
