Incomplete Documentation of Turning and Repositioning
Summary
The facility failed to ensure that the medical records for two sampled residents were complete and accurate because documentation of turning and repositioning every two hours was not found in the chart. Resident 1 was admitted and later readmitted with diagnoses including metabolic encephalopathy, acute kidney failure, and dementia. The H&P stated Resident 1 did not have the capacity to understand and make decisions, and the MDS showed the resident was dependent on staff for most ADLs and for turning and repositioning in bed. Resident 1's care plan identified a pressure ulcer on the coccyx and included interventions to encourage and assist turning and repositioning and to turn and reposition every two hours and as needed. However, the TCADL for the reviewed date had no entry under Turning and Repositioning Q2 hours and PRN, and CNA documentation regarding turning and repositioning every 2 hours was not found in the medical record. During interview, CNA 1 stated the resident needed turning and repositioning every 2 hours and as needed for wound prevention, and RN 1 stated the CNA who turned and repositioned the resident should document it in the chart. Resident 10 was admitted and later readmitted with diagnoses including Parkinson's disease, dementia, and muscle weakness. The MDS showed severe cognitive impairment and dependence on staff for bathing, toileting hygiene, lower body dressing, and partial/moderate assistance for oral and personal hygiene and turning and repositioning in bed. The care plan identified potential for skin impairment due to a history of pressure ulcers and included turning and repositioning every two hours, but the TCADL had no entries for Turning and Repositioning Q2 hours until a later date, and CNA documentation was not found for the period reviewed. The DON stated the TCADL was the facility's proof that Resident 10 was provided turning and repositioning every 2 hours and confirmed the record did not contain that documentation from admission until the later date.
