The facility failed to ensure post-fall 72-hour monitoring was completed and documented each shift for three residents reviewed for falls. Resident 1, who had a diagnosis of dementia, sustained a witnessed fall and the record did not show licensed nurses completed and documented post-fall 72-hour monitoring assessments each shift after the fall. Resident 2, who had a diagnosis of abnormalities of gait and mobility, sustained an unwitnessed fall and the record likewise did not show documented post-fall 72-hour monitoring assessments each shift. Resident 3, who had a diagnosis of dementia, sustained an unwitnessed fall and there was no documented evidence of post-fall 72-hour monitoring assessments each shift after the fall. During interview, an LVN stated falls were considered a change of condition and residents should have been monitored following the falls for any changes from baseline. The DON stated that a fall is a change of condition and, according to facility practice, licensed nurses should have documented 72-hour monitoring for each shift to identify any potential changes in condition from baseline and to ensure the physician was notified for appropriate interventions. The facility policy titled Notification of Changes stated the facility consults the resident's physician when there is a change requiring notification, including significant changes in physical, mental, or psychosocial condition and clinical complications.
