Improper handling of a resident after a fall

Summary

The facility failed to ensure nursing staff had the competencies and skills needed to care for a resident with severe cognitive impairment, dependence for ADLs, and a documented high fall risk. The resident had diagnoses including encephalopathy, schizoaffective disorder, chronic respiratory failure dependent on a respirator, tracheostomy, gastrostomy, and bilateral lower leg contractures. The MDS showed the resident was dependent for care and rolling left and right, and the fall risk evaluation identified the resident as high risk for falls. During care on the day of the incident, CNA 1 was changing the resident’s incontinent brief and positioned him on his left side with a wedge behind his back and a pillow between his legs. CNA 1 then lowered the bed and turned away to discard linens, heard a thump, and found the resident on the floor lying face down. CNA 1 stated the resident did not roll on his own and that floor mats were not in place. EMS documented that the resident had fallen from the bed to the floor and had an abrasion, a hematoma to the top of the head, and swelling and bruising to the right eye. After the fall, staff moved the resident from the floor back to the bed. RT 1 stated the resident was on his side when he entered the room and that staff rolled the resident side to side to place a sheet underneath him and then lifted him back onto the bed. RNS 2 stated the resident should have been left on the floor until paramedics arrived because moving him could have caused further harm, especially with possible fractures or internal bleeding. The facility’s fall policy stated the resident is not to be moved until assessed by the licensed nurse, and the DSD stated CNAs were trained not to move a resident after a fall and to wait for the nurse to assess the situation. RNS 1 and CNA 1 stated the resident was moved so the nurse could assess him and to apply pressure to the forehead wound.
