Failure to Monitor Antibiotic Use and Report Non-Qualifying Criteria

Summary

The facility failed to implement its antibiotic stewardship program for two sampled residents who were receiving antibiotics, and the facility did not report negative findings to the attending physician. Resident 61 was admitted with diagnoses including encephalopathy and COPD, and the MDS showed severely impaired cognitive skills and extensive assistance needs for mobility and activities of daily living. The MAR showed doxycycline hyclate was administered twice daily for fever from 5/4/2026 through 5/11/2026. During review with the Infection Preventionist, Resident 61’s UA dated 5/5/2026 showed no infection detected, and the urine culture dated 5/6/2026 showed no abnormal infectious organism growth. The IP stated there was no documented evidence that staff communicated these negative results to the physician. The resident’s McGeer’s Criteria dated 5/6/2026 was also reviewed and was not completed. The IP stated the McGeer’s Criteria should have indicated that Resident 61 did not meet criteria for antibiotics because of the negative UA and UCX results, and that there was no physician communication indicating the resident did not meet criteria for antibiotic use. Resident 89 was admitted with diagnoses including encephalopathy, pneumonia, and dementia, and the MDS showed severely impaired cognitive skills and extensive assistance needs for care and transfers. The physician order report showed azithromycin was started for pneumonia from 1/16/2026 through 1/20/2026. During review with the IP, Resident 89’s McGeer’s Criteria dated 1/16/2026 showed the criteria for antibiotics was not met because the resident did not have at least one constitutional criterion, including fever, leukocytosis, change in mental status from baseline, or acute functional decline. The IP stated there was no communication with the physician that the McGeer’s criteria was not met. The DON stated that negative lab results or McGeer’s criteria not being met should be reported to the physician so nurses will know whether the antibiotic should be continued or stopped, and that failure to report negative labs when on an antibiotic can lead to unnecessary treatment and antibiotic resistance.
