Summary

The facility failed to assess and monitor a resident who was readmitted and then transferred to a Board & Care. The resident’s record showed readmission with diagnoses including acute and chronic respiratory failure with hypercapnia, unspecified inflammatory spondylopathy of the lumbar region, and acute respiratory failure with hypoxia. The resident’s H&P described the resident as alert and oriented to person, place, and time, while the MDS indicated moderately impaired cognition. The resident’s chart did not contain an admission assessment, vital signs, nursing progress notes, or a discharge/transfer summary during the resident’s brief stay. RN 1 stated the resident arrived from the GACH at about 5:30 PM and remained at the facility for about one hour. RN 1 stated she checked the resident’s vital signs but did not document them, attempted a skin and body assessment but the resident refused, and did not document any of her encounters with the resident. RN 1 also stated she did not review the discharge medication list, discharge summary, or instructions that accompanied the resident. The DON stated the admitting nurse should conduct and document an assessment, review the hospital discharge papers, notify the physician, reconcile medications and orders, and complete discharge-related documentation when a resident is transferred or discharged. The DON also stated the nurse should have taken instruction from him rather than the SSD. The facility’s policy required transfer or discharge details to be documented in the medical record, including the basis for transfer or discharge, the date and time, the new location, mode of transportation, and a summary of the resident’s overall medical, physical, and mental condition.
