An LVN stated the resident’s care plan did not include a floor mat, and another LVN stated there was no reason for the resident not to have one and that the resident used to have a floor mat. A CNA stated no additional interventions were in place besides keeping the bed low. The DON reviewed the fall care plan and agreed the resident was high fall risk and that no additional interventions had been implemented following the documented falls. The facility policy stated that if falling recurs despite initial interventions, staff will implement additional or different interventions, or indicate why the current approach remains relevant.
