Unsafe Exit Access and Inadequate Supervision Allowed Resident Elopement

The Villas At Saratoga Skilled Nsg & Assisted LvgSaratoga, California Survey Completed on 05-08-2026

Summary

The facility failed to ensure a safe environment and adequate supervision when one resident was able to leave the building unattended on 4/7/26. The resident had diagnoses including sepsis due to MRSA, hypertension, difficulty walking, bipolar disorder, and depression. His MDS dated 3/16/26 showed a BIMS score of 15, indicating he was cognitively intact. The resident later returned to the facility intoxicated and was sent by 911 to an acute hospital for evaluation. During interviews, the DON stated the resident left the facility on 4/7/26 and returned around 10 p.m. intoxicated. The resident stated he left the facility at about 12:30 p.m., nobody saw him or stopped him, and he took three bus rides and returned with alcohol. RN A stated she was assigned to the resident that morning, that he wore a Wanderguard, and that she noticed around 1 p.m. that he had not eaten lunch and began looking for him. RN A stated she did not hear a Wanderguard alarm and reported that unlocked doors led from the resident’s area to the patio and then to an unlocked exit gate, with no staff supervising the gate. CNA B stated she was assigned to the resident that morning, took lunch break from 12:10 to 12:40 p.m., and when she returned the resident was not in his room and his lunch tray was still there. CNA B also stated she did not hear a Wanderguard alarm and that the entrance and exit doors in the resident’s area were always unlocked. The DON verified during tour that the back entrance/exit gate, the doors on either side of the gate, and multiple patio doors were unlocked without alarms, and that there were no staff supervising those doors or the gate. The resident’s progress note documented that when he returned he appeared intoxicated, had beer and a bottle of vodka, drank vodka in front of staff, and 911 was called because staff determined it was unsafe for him and other residents and staff.

D
F0689 F689: Ensure that a nursing home area is free from accident hazards and provides adequate supervision to prevent accidents.
Short Summary

Failure to Follow Fall Interventions: A resident with dementia, ESRD on dialysis, impaired mobility, and a history of falls was observed ambulating and self-transferring in her room and bathroom without staff assistance, despite orders and a care plan requiring one-person assist with a walker, call light use, frequent safety checks, and skid strips by the bed. Staff interviews confirmed the resident was transferring independently and that the skid strips were not in place as documented.

Short Summary

A resident assessed as high risk for falls did not receive the physician-ordered fall mat on the right side of the bed. Surveyors observed no mat during multiple checks, and the TAR did not reflect the intervention. The resident said the mat had been removed after a new bed was placed, while an RN and the DON acknowledged the order remained in place even though the mat was no longer being used.

During observation, the CNA lifted the resident by the upper back and moved him between the wheelchair and bed without mechanical assistance, despite having completed lift competency training and despite the DON confirming the resident required a mechanical lift for all transfers.

Failure to Supervise Resident at Risk for Elopement
J
F0689 F689: Ensure that a nursing home area is free from accident hazards and provides adequate supervision to prevent accidents.
Short Summary

Failure to supervise a resident at risk for elopement led to repeated exit-seeking and wandering events, including being found off the unit and near exterior areas. The resident had dementia with cognitive impairment, hemiplegia, and diabetes, and staff notes described missing documentation of several wandering episodes, an incomplete care plan intervention, and incidents involving a broken alarmed door, a Wanderguard, and an unknown visitor letting the resident off the unit.

Failure to verify correct sling size and safe lift use during resident transfers
E
F0689 F689: Ensure that a nursing home area is free from accident hazards and provides adequate supervision to prevent accidents.
Short Summary

A resident with severe cognitive impairment fell during a lift transfer when a sling strap came loose and the chest strap clip broke, and two other residents were observed being transferred with total body lifts using slings that were not verified against their care plans or mobility assessments. Staff interviews showed some CNAs relied on the sling already in the room or on how it fit, rather than consistently checking the care plan or Kardex for the correct sling size and transfer method.
