Summary

The facility failed to ensure that two of four kitchen staff, including the Dietary Supervisor and one kitchen staff member, followed the IDDSI testing methods for pureed foods for six residents ordered a pureed level 4 diet. During an observation, the kitchen staff member added thickener to pureed rice pilaf and performed only the spoon test; when the spoon was tilted, the food fell off right away. During a later observation, pureed pork was blended, thickener was added while mixing, and the Dietary Supervisor again performed only the spoon test; when the spoon was tilted, the pureed pork easily fell onto the plate below. During interview, the kitchen staff member stated they used the spoon test to check for lumps and expected the food to fall off the spoon when tilted. The Dietary Supervisor stated they only used the Spoon Tilt Test to test pureed food and were not aware that both the Fork Drip Test and Spoon Tilt Test were required. Review of the posted IDDSI guidelines and the facility's Dysphagia Textures - Puree IDDSI Level 4 policy showed that prepared pureed recipes should be tested prior to service and must pass both critical tests. The facility's Diet Type Report showed six of 74 residents had orders for Pureed - Level 4 diet.
