Failure to Maintain Bed Alarm Function and Licensed Nurse Coverage

Summary

The facility failed to implement adequate accident prevention interventions for Resident 71 by not ensuring the bed alarm was functioning at the time of the fall and by not providing adequate supervision. Resident 71 was admitted and later readmitted with diagnoses including a history of falling, need for assistance with personal care, and dementia. The resident’s care plan, initiated and revised before the incident, indicated use of a bed sensor pad alarm due to repeated falls and directed staff to check the bed alarm and machine every shift and keep the alarm monitor machine in hand to properly monitor the resident outside visual checks. Resident 71’s MDS showed severely impaired cognition and the need for substantial to maximum assistance with ADLs and mobility. The fall risk evaluation identified the resident as a high fall risk. The change in condition evaluation documented that the resident had an unwitnessed fall and was found seated on the left side of the bed facing the door. The resident reported attempting to go to the bathroom independently, losing balance, and falling. The evaluation noted no change in mental status, no discoloration or bruising, no skin changes, and no change in range of motion. Staff interviews and staffing records showed that both LVN 3 and LVN 4 were on meal break at the same time during the period when the fall occurred. The employee timecard report showed both nurses were off the unit from 7:05 PM to 7:35 PM, and the nursing staff assignment sheet showed staggered meal breaks were scheduled. CNA 4 stated there was no licensed nurse present at station 2, where Resident 71’s room could be monitored, and that the bed alarm did not activate. RN 1, LVN 3, and LVN 4 each acknowledged that licensed nurses were expected to stagger breaks so one nurse remained at each station, and they confirmed that both nurses leaving station 2 together created a safety concern and limited supervision at the time of the fall.
