Failure to Address Significant Unplanned Weight Loss
Summary
The facility failed to provide adequate nutritional monitoring and interventions for two residents who experienced significant unplanned weight loss. For Resident 2, the record showed a weight decrease from 105.4 lbs on 6/2/25 to 97 lbs on 7/28/25, a 7.95% loss in 56 days. The care plan was not updated to reflect the weight loss, and there were no IDT notes, RD notes, or documented interventions addressing the decline. Interviews with the resident’s daughter, the LVN, the SLP, the RD, and the DON confirmed that the weight loss was not identified or addressed in a timely manner, and the resident remained on a mechanical soft diet that the daughter reported the resident disliked and ate poorly. Resident 2’s record also showed no documented physician notification or SBAR communication related to the weight loss. The SLP stated Resident 2 was not on his patient load and that a fortified/high protein diet should have been started immediately once the weight loss was identified. The RD stated she was unaware of the 7.95% loss and relied on current weights rather than looking back at prior weights. The DON stated the RD should have identified the weight loss and that nursing was not responsible for looking for interventions, while also confirming that no RD notes or IDT notes regarding weight loss were found in the chart. Resident 4 was admitted with diagnoses including protein-calorie malnutrition, type 2 diabetes, CKD, dementia, and altered mental status. The resident’s weights showed a downward trend from 167 lbs on 5/30/25 to 151 lbs on 6/27/25, a 9.58% loss, and to 143 lbs on 7/17/25, a 14.37% loss. The record showed that nutritional interventions were delayed and initially limited to snacks, with diabetic snacks added on 7/1/25 and fortified/high protein diet interventions not added until 7/18/25, after the resident had already sustained substantial weight loss. The record also showed that the resident had variable oral intake, with intake documented at 51-75% on nutrition assessment and 25-50% in later IDT documentation. Interviews and record review showed that the RD did not trend weights back to admission and relied on weekly weights without comparing them to prior weeks, while the DON stated weights should have been trended to identify changes before they became significant. The SLP stated Resident 4 had no current swallowing or chewing issues and that a fortified/high protein diet should have been started immediately on admission or when weight loss was first noted. The DON stated Resident 4 should have been admitted on high protein shakes and a fortified high protein diet because of the admitting diagnosis of protein-calorie malnutrition, and stated that the facility’s policy and procedures regarding weight loss and weight management had not been followed.
