Delayed and Inaccurate PT ROM Documentation

Summary

The facility failed to maintain timely and accurate completion of PT Joint Mobility Screens for two residents. For Resident 3, PT JMS entries with effective dates of 4/28/2025 and 10/28/2025 were not completed and documented until 11/4/2025, and the PT JMS with an effective date of 7/28/2025 was not completed until 9/22/2025. Resident 3’s record showed diagnoses including cerebrovascular disease, dementia, and dysphagia, and the MDS indicated severe cognitive impairment, functional ROM limitations in both upper and lower extremities, and dependence for oral hygiene, toileting, bathing, lower body dressing, and bed-to-chair transfers. During observation, Resident 3 was in bed while an RNA provided PROM to both upper and lower extremities and applied knee splints and soft heel protectors. The PT stated that PT is responsible for monitoring ROM, that JMS should be completed and documented on time, and that if completed very late it may not be accurate. The PT also stated the effective date was not accurate because the assessment was not performed until the day the JMS was documented. For Resident 7, PT JMS entries with effective dates of 3/3/2025 and 6/2/2025 were not completed and documented until 4/2/2025 and 11/4/2025, respectively. Resident 7’s record showed diagnoses including cerebrovascular disease, hemiplegia, and hemiparesis following cerebral infarction, and the MDS indicated severe cognitive impairment, ROM limitation on one side of the upper and lower extremities, and dependence for oral hygiene, toileting, bathing, dressing, and bed-to-chair transfers. During observation, an RNA performed PROM to the resident’s upper and lower extremities and applied a right elbow splint, right knee splint, and right PRAFO; the RNA stated the resident could not move the right arm and leg that much.
