Summary

The facility failed to ensure food was stored and prepared under sanitary conditions. Review of facility documents showed blank kitchen cleaning and validation checklists, and the facility provided a signed statement that no completed kitchen cleaning logs existed for the prior three months. The Dietary Manager stated that the kitchen was generally cleaned after meals and that staff knew to clean when needed, but there were no completed logs available to show that routine cleaning had been documented. During interview, a CNA reported ongoing cleanliness concerns from the kitchen. She stated that cups sent from the kitchen as clean sometimes had grease residue in them, and that juice pitchers sometimes contained dirt or residue on the bottom inside. She also described an instance in which an old used meal tray was left on a meal cart with new trays that were being delivered to residents. She said she returned dirty dishes to the kitchen when she noticed the residue. Kitchen observations identified unsanitary conditions in multiple areas. A puddle of standing water was observed under the pots and pans storage rack, extending along the wall, and the Dietary Manager stated that heavy rain caused water to leak into the kitchen and that this had been an ongoing issue since he started working at the facility. In the dry goods storage area, a damaged can of tropical fruit was observed leaking sticky yellowish residue onto the floor, rack, and wheel of the rack. The next day, dried sticky residue and smeared marks remained on the floor and around the rack wheel. The Maintenance Director stated he had received a work order that morning and believed the water under the rack may have come from a nearby sink or from wet pots and pans, while the Administrator stated that food residue or spilled food left overnight would not meet her expectation and could pose a risk for infection control issues and pests in the kitchen.
