Failure to Timely Report Resident-to-Resident Abuse Allegation

Sandstone Of Tucson Rehab CentreTucson, Arizona Survey Completed on 05-07-2026

Summary

The facility failed to ensure that an allegation of resident-to-resident abuse was reported to the applicable state agencies within 2 hours for two residents. The deficiency involved a resident with dementia, bipolar disorder, heart failure, and severe cognitive impairment, and another resident with neurocognitive disorder with Lewy bodies, dementia, PTSD, chronic heart disease, and wandering and behavioral symptoms directed toward others. For one resident, a nursing note documented a small discoloration/light bruise to the right outer arm, and the resident stated she hit her arm on the door and could not recall further details. A later nursing note described that staff were assisting with redirection of the other resident during a wandering episode when that resident entered the room, became emotionally escalated, and grabbed the first resident’s right arm and right leg while she was lying in bed. Staff intervened and separated the residents, and the resident was reassured and assessed. The note also stated that police responded but would not complete a report because they did not observe the incident. For the second resident, nursing documentation showed the resident was exit-seeking, combative, and entering other residents’ rooms, and later was agitated, pacing, and hitting staff during redirection. The facility self-report stated that the resident entered the first resident’s room, yelled a male’s name, and grabbed the first resident’s arm and leg while staff attempted to de-escalate and separate the residents. Interviews with staff confirmed the resident-to-resident interaction and that staff considered the event possibly abusive. The covering administrator stated the incident occurred around 2:00 p.m. and was reported to the State Agency the next day, explaining that the delay was because there was no harm done to the residents. The facility policy stated resident-to-resident altercations should be reviewed for potential abuse, and the cited regulation required immediate reporting, but not later than 2 hours, when the event involves abuse or serious bodily injury.
