Missed Care Conferences and Failure to Update Care Plan After Seizure Disorder

Summary

The facility failed to review, revise, and implement comprehensive, person-centered care plans in accordance with resident needs. Record review and interviews showed that quarterly interdisciplinary care plan conferences were not conducted and documented for three residents. Resident #1 had diagnoses including cervical disc disorder with myelopathy, intestinal obstruction, protein-calorie malnutrition, chronic respiratory failure, and diabetes mellitus, but only two admission IDT conferences were documented and no subsequent quarterly care conferences were found. The resident stated he/she could not remember participating in a care planning meeting, and staff confirmed that no later IDT care conferences had been completed. Resident #3 had diagnoses including amputation involving multiple toes and fingers, acquired absence of right and left fingers, type 2 diabetes mellitus, and COPD. The resident stated he/she had not participated in a care planning meeting. Staff stated there were no care conferences conducted for the resident, and the SSD confirmed there were no quarterly IDT care conferences within the past year. Resident #64 had diagnoses including sepsis, acute cholecystitis, and BPH with urinary tract symptoms. The record showed one IDT conference on 7/1/25, but no other documented quarterly care conferences were found, and the SSD confirmed no subsequent IDT care conferences were conducted. The facility also failed to revise the comprehensive care plan for Resident #99 after a significant change in condition. Resident #99 had diagnoses including Parkinson's disease, atrial fibrillation, type 2 diabetes mellitus, and bipolar disorder. After a seizure-like episode, the resident was sent to the hospital, where the history and physical documented a tonic-clonic seizure and seizure precautions, and the discharge summary listed seizure disorder with levetiracetam prescribed. The LTC order recap later showed levetiracetam and then lamotrigine ordered for seizure disorder, but the MDS did not mark seizure disorder or epilepsy as an active diagnosis, and the care plan report showed no seizure interventions or tasks were initiated.
