Summary

The facility failed to ensure that accurate grievance officer contact information was available to residents and their representatives through required postings or individual notice. During observations, the grievance procedure notice posted throughout the residents’ courts identified the Grievance Officer as GO #1 and listed a mailing address, email address, and phone number for that individual. However, the Social Services Director stated that she was the current Grievance Officer, having assumed the role in November 2025, and confirmed that the posted notice still contained the former officer’s information. The facility also failed to provide clear instructions on how to file and submit grievances. The grievance forms posted on each court and outside the Social Services Office included fields for documenting a concern, investigation findings, and follow-up, but they did not include instructions for where or how to submit a completed form. The admission packets reviewed by the Social Services Director also did not include written information identifying the Grievance Officer or providing contact details, and no additional written notice of the new Grievance Officer had been provided to residents or their representatives. During the Resident Council meeting, residents stated they did not know who the new Grievance Officer was, and the council noted that the former officer had left in September without the new officer introducing herself to the council. Review of Resident Council minutes from September through December showed ongoing resident concerns about missing laundry and call light issues, but no introduction of the new Grievance Officer was documented. The facility’s grievance policy required residents to be notified individually or through prominent postings of the right to file grievances, the grievance official’s contact information, and the process for submitting grievances, but the posted notice and admission materials did not reflect those requirements.
