Unsafe Elevator Flooring and Inadequate Room Temperature Control
Summary
The facility failed to provide a safe, comfortable, and homelike environment for residents by allowing the elevator flooring to remain deteriorated and uneven. During observations from 12/14-18/25, the elevator floor was seen as approximately 6-inch square sections of linoleum-like material with curling edges, and two sections were lifting and had been patched with duct tape while remaining uneven and not fully adhered to the floor. The Director of Life Safety and Maintenance stated he had direct oversight of the elevator and acknowledged that the loose pieces posed a potential hazard to residents. The facility also failed to maintain resident bedrooms in a comfortable condition because multiple windows were allowing drafts and cold air into rooms. Residents reported that their rooms had been cold for weeks, that windows were taped up because of drafts, and that blankets, pillows, towels, and space heaters were being used to block cold air. In one room, the resident representative stated the resident complained of the room being constantly cold, and observation showed multiple layers of black tape around the window with folded blankets placed at the base, yet the draft could still be felt. Maintenance work orders documented complaints of cold rooms, drafty windows, windows that would not shut, and a heater request. Room temperature checks also showed temperatures below the facility standard of 71 to 81 degrees Fahrenheit. The Director of Life Safety and Maintenance stated weekly checks were being done in selected rooms, but there was no policy for checking room temperature, and the common area was not included in the weekly checks even though resident activities were held there. During observation, two of eight rooms checked measured 67.7 degrees Fahrenheit and 69.3 degrees Fahrenheit, and the first-floor common area measured 66.6 degrees Fahrenheit. Additional observations showed multiple rooms across several courts with windows sealed with duct tape or blocked with blankets or towels.
