Summary

The facility failed to ensure that dietary staff consistently used required hair restraints while preparing and distributing food in the kitchen. During a lunch observation, a dietary aide was seen preparing lunch trays without a mustache or beard restraint, despite having a mustache that extended beyond the upper lip area. The aide reported having worked at the facility for one and a half years and stated he had never worn a beard or mustache restraint and had not been informed about the requirement. He acknowledged that if hair fell into a resident's meal, it could cause them to choke. Interviews with facility leadership and staff confirmed that the facility’s policy required all kitchen staff and anyone entering food preparation or service areas to wear hairnets and beard nets if they had facial hair. The owner stated that dietary staff are trained by the dietary manager and that hair and beard restraints are always required in the kitchen. An RN stated that all dietary staff should wear hairnets and facial masks and that not wearing correct hair nets could be harmful to residents' health. The administrator affirmed that kitchen staff were required to wear hairnets and beard nets at all times in food preparation areas and that failure to follow these protocols could result in cross-contamination and infection. Review of the written Hair Restraint and Hair Net Compliance policy, dated 1/01/2025, showed that all dietary staff involved in food handling must wear appropriate hair restraints, including beard guards for employees with facial hair, to prevent hair contamination of food, clean equipment, and food-contact surfaces.
