Resident 4 had diagnoses including schizoaffective disorder, diabetes, and depression, and was assessed on the MDS as having moderately impaired cognitive skills and requiring supervision for oral hygiene, toileting, and showering. During interviews, RN 1 confirmed that facility practice was to conduct IDT meetings to formally evaluate or reevaluate resident care and medication regimens with the psychiatrist, and stated that an IDT meeting should have been conducted and the care plan revised after the earlier aggressive behaviors. LVN 1, who authored the 3/10/2026 SBAR, acknowledged that she did not revise Resident 3’s care plan and stated that the care plan should be revised after continued episodes of aggressive behavior. The facility’s policy on comprehensive, person-centered safety and supervision required ongoing assessments and care plan revisions when resident conditions change or desired outcomes are not met, which was not followed in this case.
