Failure to Complete Required Neuro Checks After Unwitnessed Fall

Summary

The deficiency involves the facility’s failure to provide adequate monitoring after a change in condition following an unwitnessed fall for one of three residents reviewed for falls. The resident had a history of repeated falls, displaced fractures of the cervical vertebrae, and a displaced intertrochanteric fracture of the left femur, was on hospice-focused treatment, had severe cognitive impairment (BIMS score of 7), required assistance with ADLs and transfers, and was assessed as high fall risk. On the day of the incident, a CNA heard the resident’s alarm, found the resident on the floor next to the bed, and notified a nurse. The CNA and nurse assisted the resident back to bed, and the CNA reported checking the resident’s vital signs three times before the end of the shift. According to facility staff interviews and the neurological assessment record, the facility’s protocol required neuro checks at specific intervals for 48 hours after an unwitnessed fall. For this resident, neuro checks were documented only three times within a 30-minute period on the evening of the fall, with no further neurological assessments completed afterward. The night-shift charge nurse stated she was not informed that the resident had fallen and therefore did not perform neuro checks, despite later being notified that the resident was crying out in pain and showing a difference in leg length. Both the PM and NOC nurses acknowledged that neuro checks are important after unwitnessed falls due to possible delayed effects, and the DON stated that neuro checks are used to detect potential head injury and neurological deficits. As a result, the resident had the potential to have injuries related to the fall go unnoticed and to receive delayed care.
