Summary

The facility failed to ensure that a significant change in condition was promptly reported to licensed nursing staff for one resident when a CNA did not report a new, persistent cough for approximately four days. The resident had been admitted with acute bronchitis and COPD and, per the most recent MDS, had intact cognitive skills and required partial assistance with most ADLs and supervision for eating. During an observation and interview in the resident’s room, the resident was seen coughing forcefully and was briefly unable to speak; she then reported having the cough for a few weeks, believed she was secreting green mucus the previous day, and stated she was not receiving any treatment for the cough at that time. The resident also reported a prior hospitalization a few months earlier for bronchitis. In an interview, the CNA stated she first noticed the resident coughing after assisting with a shower several days earlier and knew the resident had previously gone to the hospital for a persistent cough in January, but she did not report the current cough to a licensed nurse because she assumed the nurses were already aware. The Charge Nurse confirmed that the resident had been sent to the hospital in January for evaluation of a persistent cough and that the EMR and verbal reports were the expected methods for communicating and documenting changes of condition. The Charge Nurse verified that no one had reported the new onset cough and that the EMR contained no documentation of a new cough since the January episode. Review of the facility’s “Changes in Resident Condition” policy indicated that changes in resident status are to be communicated through the 24-hour report management system and documented as revisions when they affect the care plan, which did not occur in this case.
