In a subsequent interview, the DON stated that oxygen tubing needs to be changed weekly and acknowledged that the facility had missed changing the tubing for this resident. This sequence of observations and interviews shows that the resident’s oxygen tubing was not changed according to the weekly physician order or the facility’s stated practice.
