During multiple days of survey observations, surveyors did not see any daily nurse staffing postings on the nursing units or at the receptionist’s desk. Group and individual interviews with alert and oriented residents revealed that they did not know where to find information about daily nurse staffing or how to determine how many staff members were working. A visitor who reported visiting regularly also stated she did not know where the nurse staffing information was located and had not seen it posted. When interviewed, the Administrator was unsure where the nurse staffing information was posted and referred the surveyor to the Assistant Administrator. The Assistant Administrator indicated that staffing information was posted on a bulletin board in the Human Resources hall and showed an “as worked” schedule for a single shift and date, and then acknowledged that the posting at the receptionist’s desk should include the facility name, shift, nursing positions, and census. The receptionist produced an empty picture frame from under the desk, stating they were replacing the frame where the posting was usually kept. The Assistant Administrator did not know if postings were retained for 18 months. The staffing coordinator stated she usually posted the nurse staffing daily and that prior postings were available, but was unsure if they covered the past 18 months. Records provided showed postings only from late August to early March, with numerous missing dates and incomplete information, including missing census data and blank RN time slots on certain shifts, despite facility policy requiring daily posting for each shift of the number of nursing personnel providing direct care.

During observations on 6/1/2026, staffing information dated 6/1/2026 was posted at the main entrance, but the census listed as 103 did not match the Resident Listing Report census of 102. Later that same day, staffing information dated 6/2/2026 was observed posted, and on 6/2/2026 staffing information dated 6/3/2026 was posted at the main entrance. On 6/3/2026, staffing information dated 6/4/2026 was posted at the 100 Hall nursing station near the off-hours facility entrance. During interview, the DON confirmed the staffing information posted was dated 6/4/2026 and stated the scheduler would prepare the staffing information sheet before leaving and corrections would be made the next day. The DON also stated the staffing sheet should not be posted in the front and should be placed behind the current day until the new shift starts. Administration stated the staff coordinator should not post the staffing information the day before and should post the current staffing information for the day. The Staffing Coordinator stated she had been putting the staffing information out before leaving at the end of the day for the following day and did not notice the census was incorrect.

During interview, NA-A verified the posting was dated 5/26/26 and stated it was important for families and visitors to see how many staff were working. SC-B stated she changed the nurse staffing hours on Thursdays for the coming week but did not update the posted sheets, and instead made the changes in the computer after they occurred when she returned on Mondays. A review of the next week's schedules showed the census as 24, and one schedule dated 5/30/26 had zeros for all staff listed with a census of 24. The DON verified the nurse staff posting should be posted daily so anyone could see how many residents were in the facility and how many staff were working, and stated it was important so families could have confidence there was enough staff to take care of their family members.
