Hot Water Temperatures Not Monitored in Resident Bathrooms

Summary

The facility failed to monitor water temperatures to ensure the environment was free from accident hazards. Policy review showed the facility had an Accident and Hazard Prevention Policy requiring a systematic process to identify, evaluate, and correct environmental hazards on an ongoing basis, and the Maintenance Director job description required regular safety inspections and documentation. However, no documentation was found showing water temperatures were monitored from 11/26/2025 through 3/15/2026, and a supplier invoice dated 3/16/2026 documented a response to hot water issues, including cleaning the mixing valve, replacing a casing gasket, and setting the temperature to 110 degrees F. During observations, hot water temperatures were found in multiple occupied resident bathrooms. Resident #4’s bathroom sink measured 132 degrees F, Resident #27’s sink measured 132 degrees F, Resident #7 and Resident #30’s shared bathroom sink measured 134 degrees F, Resident #26’s bathroom sink measured 133 degrees F, Resident #14 and Resident #15’s shared bathroom sink measured 134 degrees F, and Resident #20 and Resident #23’s shared bathroom sink measured 132 degrees F. These temperatures were observed in bathrooms assigned to residents with varying levels of cognitive and physical impairment, including residents who were cognitively intact, moderately impaired, or severely cognitively impaired, and several who required assistance with toileting and personal hygiene. Interviews showed several residents did not routinely use their bathrooms, while others required staff assistance for bathing or toileting. The DON stated some residents were bathed by CNA staff, hospice, or in the shower room, and that certain residents did not use their restroom. The Executive Director/Maintenance Supervisor stated water temperatures should be checked weekly and later stated weekly checks should be done in random rooms and different hallways, while the Administrator stated there was no policy on the frequency of checks and said daily checks would be done for the next 7 days and then weekly after that.
