Summary

The deficiency involves the facility’s failure to report an allegation of misappropriation and/or medication diversion of a resident’s prescribed Mounjaro injection to the State Survey Agency as required. A cognitively intact resident with type 2 diabetes mellitus, admitted in November 2025 and ordered to receive Mounjaro 5 mg subcutaneously once every 7 days, reported not receiving a scheduled Mounjaro injection on a Friday later identified as 3/13/2026. The resident stated that when this was reported to nursing staff on Sunday, staff responded that the medication had been documented as administered and identified an LPN as the nurse who signed for the dose. Facility records showed that four Mounjaro pens were delivered on 3/6/2026 and were being tracked in the narcotic count book due to their high cost. On 3/15/2026, after the resident’s report, nursing staff reviewed the narcotic count and confirmed that four pens remained in the refrigerator, consistent with the original delivery, and the narcotic record did not show removal of a dose on 3/13/2026. On 3/16/2026, the ADON confirmed three pens were present and documented, after administering a dose that day, and identified that the LPN had documented administration of a dose on 3/13/2026 that was not reflected in the narcotic count when reviewed on 3/15/2026. During a later interview, the LPN could not explain the discrepancy between the documented administration and the unchanged narcotic count through 3/15/2026, nor the subsequent decrease in count after his documentation. The DNS and Administrator were unable to provide evidence that this allegation of potential misappropriation was reported to the State Survey Agency.
