Summary

The facility did not ensure adequate supervision and assistive devices were in place to prevent accidents for two residents. Facility policy on bed safety and bed rails required consideration of resident safety, medical conditions, comfort, freedom of movement, and alternatives before use of side or bed rails, including grab bars, roll guards, foam bumpers, lowering the bed, or a concave mattress. Resident R69 had a BIMS score of 12, indicating moderate cognitive impairment, and a fall risk score of 12, indicating moderate fall risk. An interim bed rail evaluation documented that the resident was ambulatory, had used bed rails in the past for bed mobility and a sense of security, and requested left and right grab bars. On observation, the resident was found in bed with a bruise to the right eye and stated she had rolled out of bed during the night and fell, resulting in injury to the right eye. The hospital discharge summary documented a head CT showing small hemorrhagic contusions and a soft tissue hematoma. The room observation showed no grab bars on the bed, and the DON stated it was missed to communicate with maintenance to place the bed rails. Resident R117 had diagnoses including paraplegia, COPD, and chronic pain syndrome, and was cognitively intact with a BIMS score of 15. The resident’s care plan identified fall risk related to deconditioning, and the MDS coded the resident as dependent for rolling left and right. During care, a CNA left the resident lying on her side in bed to get the wound nurse, believing the resident was safe with pillows in place. While the CNA was out of the room, the resident slid off the bed and was found on the floor between the beds, bleeding from the head. The resident stated she began to slide off the bed and could not prevent it due to paraplegia. The DON confirmed the CNA left the resident lying on her side in bed when she walked out of the room.
