The resident was on a heart-healthy, full liquid diet, and intake was to be tracked by nursing assistant staff using an “amount eaten at meal” task form. Review of this documentation showed missing recorded percentages for breakfast and lunch on multiple dates in March 2026, specifically the 18th, 21st, 22nd, 23rd, 25th, and 27th. The RD confirmed that intake was to be documented in this manner, and the DON confirmed that the “amount eaten at meal” task form had not been completed for those meals on the identified dates. These omissions and inaccuracies demonstrated that the facility did not maintain medical records in accordance with accepted professional standards.
