Summary

The deficiency involves the facility’s failure to store food and maintain food service equipment in accordance with its own policy and professional standards in all five nourishment areas. The facility’s policy "Food from Home- Safety" required safe and sanitary storage, handling, and consumption of all food, including items brought by families and visitors, with staff appointed to check resident refrigerators for proper temperatures, food containment and quality, disposal of items per policy, and proper labeling and dating, including discarding leftovers after three days. Despite this, surveyors observed in the M1 pantry refrigerator undated pizza with visible green mold, an undated container of sour cream, and an unlabeled, undated container of soup. Further observations showed similar failures in other nourishment areas. In the M2 pantry refrigerator, surveyors found a bag of sliced cheese past its sell-by date, multiple undated containers of food and desserts, unlabeled Styrofoam takeout containers and grocery bags of food, an undated egg salad sandwich, and yogurt from an outside source not labeled with a resident’s name. In the M3 pantry freezer, there was a box of takeout pizza from several months prior, and the M3 and M5 pantry ice machines had black and orange spots on the white area under the door that could be wiped away, indicating they were not kept clean. In the M5 pantry freezer and refrigerator, surveyors found expired ice cream from an outside source, multiple undated and unlabeled food items, and a heavily soiled refrigerator with brown liquid spilled on the top shelf. In the M4 pantry freezer and refrigerator, there were foil-wrapped foods with resident names but no dates, expired sherbet, undated and improperly sealed ice cream items, and multiple candy and pastry items without resident names or dates, some past their best-by date. The Nursing Home Administrator stated his expectation that expired items be discarded, foods labeled and dated per policy, and equipment kept clean, which contrasted with the survey findings.
