Another care plan addressing inappropriate behavior documented that this resident preferred bed baths and refused showers. These findings, based on record review, staff and resident interviews, and documentation audits, demonstrate that the facility did not consistently provide ADL care, particularly bathing, in alignment with residents’ assessed needs and expressed preferences.
