No resident-specific information, medical history, or clinical conditions were mentioned in relation to this deficiency. Interviews with staff revealed that the Scheduler prepared daily nurse staffing sheets and typically placed them in a binder at the nurses' station on Fridays for weekend use. The Scheduler stated she had last worked on Thursday and had given the completed staffing sheets for the following three days to the Assistant Director of Nursing (ADON), who reported placing them in a green staffing binder at the nurse's station and relying on nurses to ensure they were current and posted. The DON stated that the charge nurse was responsible for ensuring the daily nurse staffing sheets were posted, and confirmed that the nurse interviewed was the charge nurse that weekend. The Administrator stated the Scheduler was responsible for completing the sheets Monday through Friday, and nurses were responsible for posting them on weekends, confirming that the charge nurse should have updated and posted the current staffing information where it was clearly visible to residents and visitors.

During interview, the DON confirmed the staffing information posted was dated 6/4/2026 and stated the scheduler would prepare the staffing information sheet before leaving and corrections would be made the next day. The DON also stated the staffing sheet should not be posted in the front and should be placed behind the current day until the new shift starts. Administration stated the staff coordinator should not post the staffing information the day before and should post the current staffing information for the day. The Staffing Coordinator stated she had been putting the staffing information out before leaving at the end of the day for the following day and did not notice the census was incorrect.

During interview, NA-A verified the posting was dated 5/26/26 and stated it was important for families and visitors to see how many staff were working. SC-B stated she changed the nurse staffing hours on Thursdays for the coming week but did not update the posted sheets, and instead made the changes in the computer after they occurred when she returned on Mondays. A review of the next week's schedules showed the census as 24, and one schedule dated 5/30/26 had zeros for all staff listed with a census of 24. The DON verified the nurse staff posting should be posted daily so anyone could see how many residents were in the facility and how many staff were working, and stated it was important so families could have confidence there was enough staff to take care of their family members.
