During interview, the resident reported previously seeing a counselor at another facility due to bipolar disorder and stated that no mental health practitioner had been seen since admission. In a separate interview, the Social Services Director confirmed that the resident had not been referred for mental health services and acknowledged that such a referral should have been made, demonstrating that the planned intervention to arrange mental health services was not implemented.
