Summary

The facility failed to complete a comprehensive investigation after a resident fell from a chair and sustained a minor injury. The resident had multiple diagnoses including heart failure, depression, anxiety, arthritis, auditory and visual hallucinations, dementia, and muscle weakness. The resident’s MDS showed significant cognitive incapacity and need for supervision with transfers and mobility, and the care plan identified the resident as being at risk for falls related to confusion. According to the nursing notes and incident report, the resident was found lying face down on the floor after an unwitnessed fall from a chair while asleep and leaning forward.
