Summary

The deficiency involves the facility’s failure to ensure staff consistently implemented Enhanced Barrier Precautions (EBP) in accordance with CDC guidelines for a resident with an indwelling urinary catheter. The resident’s admission record documented benign prostatic hyperplasia with lower urinary tract symptoms, and orders and care plan confirmed the presence of an indwelling Foley catheter. The medical record did not contain evidence that staff were knowledgeable about the resident’s need for EBPs. Facility policy, revised 8/8/24, stated that EBPs would be initiated for residents with urinary catheters and that PPE use was required for high-contact care activities such as transfers, assisting with toileting, and personal hygiene. The DON stated that staff were expected to gown and glove when transferring and assisting a resident on EBPs to the bathroom, confirming that the observed practice did not meet facility expectations or policy requirements.
