Food and Nutrition Services Staffing Qualifications Not Met

During interview, the dietary manager stated she did not have a Certified Professional Food Manager Certification and was not a Registered Dietitian, and she said she had only been at the facility for a short time and had not yet obtained the certifications. She also stated the facility had a Registered Dietitian. In a phone interview, the Registered Dietitian stated she came into the facility bi-weekly, with two visits per month, and also completed remote assessments, and said she was a consultant for the corporation. The nursing home administrator stated the Registered Dietitian was full-time for the facility, but also stated the dietary manager had been employed for 6 weeks. Review of the Registered Dietitian’s invoices showed a total of 26.75 billed hours for December 2025 through February 2026 combined, with 6.25 hours in December, 10 hours in January, and 10.5 hours in February. The nursing home administrator also stated she did all of the food ordering and pretty much ran the kitchen because the dietary manager was so new.

Although she completed a UF nutrition and foodservice training pathway, she told surveyors she had not taken or scheduled the CDM exam and had no degree in Nutrition, Hospitality, or Food Service. The deficiency affected 84 of 84 residents receiving meals from the department.

During interview, the Culinary Manager stated she had worked at the facility since August 2025 and was overseen by the facility’s RD, who worked one day per week. Documentation of the Culinary Manager’s certification was requested, but no certification was provided on multiple occasions. When asked about her background, the Culinary Manager stated she had two years of experience as a cook and one year of experience as a kitchen manager at a boarding school. The CEO later confirmed the Culinary Manager did not have the required qualifications and was in the process of getting certified.

She also stated that she had completed the CDM course in September 2025 and had worked in the position for eight years managing the Food Service Department. The Administrator acknowledged that the Dietary Supervisor had been in training but had not completed it, and said she had followed up and encouraged completion of the training. The report states that this affected 84 of 84 residents receiving meals from the facility’s Food Service/Dietary Department while it was under the direction of an unqualified individual.
