Summary

The facility failed to maintain an infection control program designed to provide a safe, sanitary, and comfortable environment and help prevent the development and potential transmission of communicable diseases and infections. The deficiency involved Resident #106, who was admitted in June 2025 with diagnoses including acute respiratory failure with hypoxia. The resident’s MDS indicated cognitive intactness with a BIMS score of 15 out of 15 and dependence on staff for activities of daily living. Physician’s orders directed that the nebulizer tubing, mouthpiece, and bag be changed weekly on Sundays, and that DuoNeb inhalation solution be administered three times daily and as needed. During observations on 3/17/26 and 3/18/26, the surveyor found the resident’s nebulizer mouthpiece and the chamber holding clear liquid residual medication resting directly on the bedside nightstand and open to air, with an undated clear plastic bag folded on the nightstand behind the machine. During interview, Nurse #2 stated the equipment should have been cleaned out to remove residual medication and stored in a dated plastic bag when not in use. The Unit Manager stated the nebulizer equipment should have been cleaned after each treatment, then stored in a clean bag that should be dated. The DON also stated the nebulizer equipment should have been cleaned and stored properly.
