Summary

The facility failed to provide adequate pain management for one resident with intact cognition who was admitted with a displaced supracondylar fracture of the right femur and a periprosthetic fracture around the right knee prosthesis after surgical intervention. The resident’s admission pain assessment documented severe pain in the right posterior thigh and right popliteal region, both rated 7 on a 1-10 scale, with aching, sharp, and stabbing pain. The resident’s hospital discharge summary and physician orders included oxycodone IR 5 mg every 4 hours as needed for breakthrough pain, along with acetaminophen as needed. After admission, the resident reported severe pain and restlessness. Nursing notes documented severe pain overnight, and the resident asked for oxycodone, but the medication did not come on the overnight run. The resident received acetaminophen at 10:00 P.M. and again at 4:00 A.M., and later at 6:00 A.M., but continued to report severe pain with no relief. The narcotic log showed the resident did not receive the prescribed oxycodone until 12:00 P.M., which was 19 hours after admission. Staff interviews showed the facility did not obtain the narcotic prescription in a timely manner and did not use the on-call process to reach the physician. The ADON said she texted the physician but did not receive a return call. The admitting nurse and overnight nurse both said they did not contact the physician to update him on the resident’s pain status while waiting for authorization to obtain the oxycodone. The physician said he did not receive a message about the needed prescription, and NP #1 said staff should have used the after-hours phone number. The DON said staff should have utilized the on-call system and that the resident should not have gone 19 hours without appropriate pain medication.
