Failure to Document and Care Plan Elopement Risk

Summary

The facility failed to develop and implement a person-centered care plan with measurable objectives and timetables for one resident who had been determined by staff to be at risk for elopement. The resident was admitted in October 2023 with diagnoses including bipolar disorder, alcohol abuse, alcoholic cirrhosis of the liver with ascites, and liver cancer. A Minimum Data Set assessment dated 1/23/26 showed severe cognitive impairment with a BIMS score of 7 out of 15, and the healthcare proxy was activated on 1/22/26 because of cognitive decline. The medical record did not show documentation that the resident was at risk for wandering or elopement. The nursing admission assessment indicated the resident was not at risk for wandering, and social service progress notes, nurse practitioner notes, physician notes, and nursing progress notes from January 2026 through 3/13/26 did not indicate wandering behavior or elopement risk. Current physician orders in March 2026 did not include use of a wander guard, and the care plan on 3/13/26 did not identify elopement risk or the need for a wander guard device. Staff interviews described that the resident had been found outside the facility or in the parking lot in the early morning hours, and the resident was later wearing a wander guard on the left leg. Family reported being notified after the resident was found in the parking lot, and the resident stated the device had been placed on the leg and wanted it removed. CNA staff gave differing accounts about why the device was used, including unsupervised attempts to go outside and smoking outside unsupervised. The UM and DON both stated they could not locate documentation showing the resident exited the facility, and both said there was no elopement risk assessment or updated care plan documenting wander guard use.
