Failure to Post Required Nursing Staffing Data

On 1/5/26, surveyors observed a dry erase board on a nursing unit that displayed the date, unit census, the ratio of GNAs and nurses to residents for the day shift, and the day shift assignments for GNAs and nurses. The board was clear, readable, and in a prominent place accessible to residents and visitors, but it did not display the facility name, the total number and actual hours worked by licensed and unlicensed nursing staff per shift, did not differentiate RNs from LPNs, and did not include information for other shifts. A second observation on 1/9/26 found the same information still posted. During an interview on 1/9/26, the DON stated the facility posted assignments on the board and on the wall, and described the information as staff assignments, special compliance items, group assignments, the patient ratio for GNAs and nurses, the date, and the shift. When the surveyor asked to see where the required staffing information was posted, the DON showed the dry erase board and then the schedule in the back of the nurse's station. The DON confirmed that the facility name was not on the board and that the total number and actual hours worked by RNs, LPNs/LVNs, and CNAs were not included. She also confirmed that the schedule in the back of the nurse's station was not visible to visitors or residents and that the whiteboard also did not include the required facility name or staffing totals.

During observations on 6/1/2026, staffing information dated 6/1/2026 was posted at the main entrance, but the census listed as 103 did not match the Resident Listing Report census of 102. Later that same day, staffing information dated 6/2/2026 was observed posted, and on 6/2/2026 staffing information dated 6/3/2026 was posted at the main entrance. On 6/3/2026, staffing information dated 6/4/2026 was posted at the 100 Hall nursing station near the off-hours facility entrance. During interview, the DON confirmed the staffing information posted was dated 6/4/2026 and stated the scheduler would prepare the staffing information sheet before leaving and corrections would be made the next day. The DON also stated the staffing sheet should not be posted in the front and should be placed behind the current day until the new shift starts. Administration stated the staff coordinator should not post the staffing information the day before and should post the current staffing information for the day. The Staffing Coordinator stated she had been putting the staffing information out before leaving at the end of the day for the following day and did not notice the census was incorrect.

The facility policy, Nurse Staffing Posted Information dated 02/01/2026, stated that nurse staffing information was to be made readily available in a readable format to residents, staff, and visitors at any given time.

Surveyors observed the posting on multiple occasions and found it dated 5/26/26 on 5/27/26 at 2:15 p.m. and again on 5/28/26 at 6:58 a.m., dated 5/26/25 on 5/27/26 at 4:10 p.m., and dated 5/28/26 on 5/29/2026 at 8:13 a.m. The deficiency affected all 24 residents residing in the facility. During interview, NA-A verified the posting was dated 5/26/26 and stated it was important for families and visitors to see how many staff were working. SC-B stated she changed the nurse staffing hours on Thursdays for the coming week but did not update the posted sheets, and instead made the changes in the computer after they occurred when she returned on Mondays. A review of the next week's schedules showed the census as 24, and one schedule dated 5/30/26 had zeros for all staff listed with a census of 24. The DON verified the nurse staff posting should be posted daily so anyone could see how many residents were in the facility and how many staff were working, and stated it was important so families could have confidence there was enough staff to take care of their family members.
