Summary

The deficiency involves the facility’s failure to ensure that assistance with activities of daily living (ADLs) was documented as provided for multiple residents in accordance with professional standards and each resident’s care plan. One resident’s care plan dated 02/18/2026 indicated a need for staff assistance with oral care, eating, and showering. However, review of this resident’s February and March 2026 Documentation Survey Reports showed no documented evidence that bathing or eating assistance was provided on specified day shifts, and there was no alternate documentation in the clinical record to support that these ADLs were provided or that required documentation was completed. A second resident’s care plan, revised on 03/04/2026, showed the resident required staff assistance with meals, oral care, and toileting/incontinence care. The February 2026 Documentation Survey Report revealed multiple day, evening, and night shifts with no documented evidence of eating assistance, oral hygiene assistance, or toileting assistance, and no alternate documentation in the clinical record to show that these ADLs were provided or recorded. A third resident, who required staff assistance with ADLs due to limited mobility, also had missing documentation in the January 2026 documentation survey report for bathing, eating assistance, oral hygiene, and toileting/incontinence care across multiple shifts and dates, with no alternate documentation in the clinical record. In interviews, the Administrator and the Director of Nursing confirmed that the ADLs for these residents were not documented as performed or refused and acknowledged that they should have been documented.
