For one resident, Seroquel 25 mg twice daily was started for major depressive disorder and anxiety, but no baseline assessment was completed when the medication was ordered. The DON confirmed the resident was observed repeatedly reaching toward invisible objects from the floor, stated this occurred all the time, and acknowledged the facility was not tracking that behavior or identifying an effective intervention related to it. For another resident with diagnoses including schizoaffective disorder, bipolar disorder, anxiety, and depression, Lurasidone was started for bipolar disorder and later increased, but the facility's behavior and psychotropic medication reviews did not include that antipsychotic. A regional clinical nurse confirmed quarterly psychotropic assessments were not completed for the medication. For a third resident with psychiatric diagnoses including anxiety, depression, psychotic disorder, and schizophrenia, the pharmacist recommended a gradual dose reduction of Mirtazapine after determining the condition was stable, but the LPN/MDS Coordinator did not communicate the recommendation to the psychiatrist or attending physician, and the DON stated she had not contacted the physician about it.
