Summary

The facility failed to prepare mechanical soft diet meals in a form designed to meet individual resident needs for 5 of 5 residents reviewed for this diet type (R2, R15, R17, R22, and R25). During a lunch observation on 12/3/25, the barbeque riblets served on mechanical soft trays were observed without barbeque sauce; the meat was dry ground barbeque riblets with only a small drizzle of ketchup on top. The report states that R2 had a mechanical soft/nectar thickened liquid diet with extra sauce/gravy, R15 had a no added salt diet with ground moist meat and thin liquids, R17 had a no added salt/ground meat/thin liquid diet with minced and moist meats, R22 had a controlled carbohydrate diet with mechanical soft and thin liquids, and R25 had a regular mechanical soft diet with thin liquids. The recipe for the ground barbeque pork riblette required the prepared riblets to be placed in a food processor and then put in a steam table pan with barbeque sauce or prepared broth to keep them moist. The Dietary Manager stated that barbeque sauce was not available and that ketchup was added to the top of the meat on the mechanical soft trays. The Speech Language Pathologist stated that mechanical soft diet meats need gravy or sauce on top or mixed in, rather than dry minced meat, to assure the resident is safe from aspiration while eating. The Entree Representative also stated that the mechanical soft lunch meal was not prepared as it should have been per the recipe and that the barbeque riblette meat for the mechanical soft diets was served too dry.
