Unsafe Mechanical Lift Transfer Leading to Resident Fall

Summary

The deficiency involves the facility’s failure to ensure a safe mechanical lift transfer for a resident, resulting in a fall. The resident had chronic respiratory failure, morbid obesity, anxiety disorder, no cognitive impairment, and was assessed as high risk for falls. According to the incident report, staff heard a loud noise and a scream and found the resident on the floor, on her head, with her legs still elevated in the mechanical lift sling and the top sling straps detached from the lift, while the resident remained in the wheelchair. During a subsequent observation, the resident, who was alert and pleasant in bed, showed a large purplish bruise on the anterior mid upper arm and stated she fell while being transferred in the mechanical lift because the sling came off, ending up on the floor with her wheelchair and being sent to the hospital before returning to the facility. Staff interviews revealed that two CNAs were performing the transfer from wheelchair to bed using the mechanical lift after the resident requested to return to bed. One CNA reported applying the leg straps while the other applied the arm sling. During the lift, the resident, seated in the wheelchair, was pushing back and stating that something was hurting her. One CNA, positioned in front of the resident, stated that when the resident complained of pain, the transfer should have been stopped and the straps rechecked, but this was not done before the resident tipped backwards in the wheelchair and fell. The other CNA, positioned behind the resident, reported that the resident was agitated and pushing back and that she noticed the arm sling was loose just before it detached, causing the resident to fall backwards with the wheelchair. The resident’s care plan required use of a mechanical lift for transfers and directed staff to place the sling under the resident and secure it properly before initiating the lift, ensure the sling fit securely and comfortably, and communicate clearly with the resident to ensure comfort and reassurance during the process.
