Despite the facility’s policies requiring that controlled substances be reconciled upon receipt, administration, disposition, and at the end of each shift, and that oncoming and off-going nurses visually inspect drug packages and verify counts before signing, the narcotics accountability record had been signed as accurate even though the medications were not visible on the cart at the time of the count.
