Summary

The deficiency involves a failure to ensure accurate insulin administration for a resident with diabetes mellitus. The resident, admitted on 10/10/25 and care planned for diabetes and insulin use, had a blood glucose reading of 460. The nurse (V15, LPN) contacted the nurse practitioner (V74), who ordered a one-time dose of 12 units of insulin lispro subcutaneously. The order summary reflected this 12-unit one-time dose for diabetes mellitus. During a medication administration observation, V15 prepared the insulin dose and presented the syringe to the assistant director of nursing (V72) for verification, and also showed the syringe to the surveyor. At that time, there were 13 units of insulin in the syringe instead of the ordered 12 units. V15 then proceeded toward the resident’s room and was about to administer the insulin when the surveyor requested that V15 and V72 recheck the dose. Upon re-verification, V72 identified the incorrect amount and removed 1 unit from the syringe to match the ordered 12 units. The DON (V2) later stated that insulin is a significant medication that can cause adverse reactions if given improperly and emphasized the importance of following physician orders when administering medications. The facility’s injectable medication administration policy requires medications to be administered via subcutaneous, intradermal, and intramuscular routes in a safe, accurate, and effective manner.
