The unwitnessed fall incident report documented that the resident was found lying on the floor in front of the nurses’ station with skin tears on the arms after reporting that he slid on the floor and hit his head slightly. The fall nurse acknowledged the resident had multiple falls and that the identified intervention related to his fall was treatment for chronic anemia, without additional fall‑specific interventions described in the report.
